Comparative characteristics of methods of surgical treatment of obstructive ileus colon cancer
The results of surgical treatment of 83 patients with colon cancer complicated with obstructive obstruction. Distinguish 3 degrees of impaired patency of the colon. Surgical treatment depended on the area of tumor localization in the colon obstruction degree, severity and patient's general condition associated diseases. Overall postoperative mortality was 12% (10 patients died out of 83 operated). Resection of the colon and subtotal colectomy with simultaneous reduction of the continuity of the intestine can be recommended to patients in the compensated stage of obstruction in the area of the tumor and the patient's condition stable

The modern concept of treatment of patients with giant postoperative ventral hernias
Despite the rapid development of minimally invasive endovideosurgical technologies in abdominal surgery, the number of surgical interventions using the traditional laparotomic approach remains high, after which in 4-18% of cases there are postoperative ventral hernias (PVH), and in the general statistical structure of all ventral hernias, PVH is 20-26% [3,40,47,62]. In the structure of all abdominal hernias, large incisional hernias (LPOVH) occupy a special place and account for from 3 to 14% [11,14,46,61]. About 60% of patients with POVH are aged 21 to 60 years, i.e. are a significant part of the country's able-bodied population [7,11,61].

Contracture remodelling of lower limbs sceletal muscles among rats of before reproductive and reproductive age under conditions of experimental hyperuricemia
In the experiment with white rats of before reproductive and reproductive age there were studied the features of lower limbs skeletal muscles remodeling under conditions of experimental hyperuricemia. It was defined that contracture remodeling during all terms of experiment had one-way and progressive character, which we associate with ischemia as the result of structural blood vessels reorganization due to the effect of uric acid. The contracture and cytolysis remodeling of skeletal muscles in conditions of hyperuricemia among animals of before reproductive age can be observed mainly in femoral-popliteal segment as hypertrophic thickening of intima and media, while among animals of reproductive age can be mostly seen in shin group of muscles where hypertrophic type of remodeling combines with inflammation and sclerotic changes - that can be considered the basis of sclerotic alternations in arteries

Acute esophagogastroduodenopathy in traumatic disease. Morphogenesis and private pathology of erosive damages
In the study of biopsies obtained during fibrogastroduodenoscopy (43 men, 268 biopsies) and autopsy (9 people) affected by severe concomitant injury that centralization of blood circulation at the level of the digestive system that leads to local circulatory hypoxia with morphological changes ranging from mild reversible ischemia to necrosis were detected. Polietiology and monopathogenesis of shock in general and traumatic disease in particular, mean flexibility of mechanisms of alterative components development. Morphologically stressful, immunopathological and circulatory changes are expressed in the development of acute esophagogastroduodenopathy in form of acute erosions and ulcers with severe hemorrhagic component. Clinically it is manifested in the form of dynamic ileus, dyskinesia of the stomach and duodenum, pyloric insufficiency of duodenal bile reflux

The influence of melatonin on the functional state of the hepatobiliary system with dexamethasone-induced diabetes
The influence of melatonin on the functional state of the hepatobiliary system with dexamethasone-induced diabetes mellitus was studied. The analysis of numerous research studies made it possible to talk about the high prevalence of complications in diabetes as a disease of the liver, the principal organ of metabolic processes in the body. It was established during the experiment that diabetes reduced the intensity of bile, the number of total bilirubin in it, mainly due to its conjugated forms, cholates and cholesterol, which disturbed the relationship between the components of the bile. The injection of melatonin at a dose of 10 mg / kg intraperitoneally during 10 days lead to an increase in the intensity of bile secretion, increase the concentration of bilirubin in it, both general and conjugate fractions, cholates and cholesterol, improved the processes of conjugation, normalized cholate – cholesterol coefficient.

Modern approaches to treatment of chronic renal arterial ypertension
Objective: to estimate the comparative aspect of the influence of amlodipine and Nebilet for remodeling heart patients with CKD stage III with moderate nephrogenic hypertension. Materials and methods: The study included 40 patients with renal arterial hypertension, which developed due to chronic kidney disease. Results: The study found patients with CKD stage III with moderate nephrogenic hypertension has a violation of intracardiac hemodynamics. Conclusions: These data show an improvement of intracardiac hemodynamics in all patients after 3-month therapy with amlodipine and nebilet.

Determining the viability of part of the intestine strangulated hernia at complications
The device developed by the authors for determining the non-viability of strangulated section of the small intestine is applied in 25 patients with strangulated hernia of the anterior abdominal wall. The method is based on the principle of the frequency deviation of the quartz crystal oscillator due to intestinal peristal-sis, causing a change in the distance between the electrodes, therefore, and their capacity. In 14 patients with strangulated loop of intestine is recognized as unviable, which is confirmed by developed device and re-quired resection on the margin due according to specified device. When restoring the continuity of the intes-tinal tract, preference is given for "side to side" entero-enteroanastomosis. In the postoperative period three elderly patients died due to comorbidities. In all cases, the study of necrosis boundary of gut was made cor-rectly, making the postoperative period uncomplicated considering the abdominal cavity

Preventive ultrasound imaging in pregnant women with the expected difficulties in carrying central neuraxial block
The article presents the results of a study 39 pregnant patients with the expected difficulties in carrying out the central neuraxial blockade. All the patients were divided into 2 groups. In the first (main) group, which included 20 patients used preventive ultrasound imaging of intervertebral spaces. In the second (con-trol) group, which included 19 studied patients, the optimal level of the intended puncture and the projection of the spinous processes of the lower thoracic and lumbar spine was determined by the clinical method. Ac-cording to research the use of preventive ultrasound of the lower thoracic and lumbar spine provides a fairly reliable guidelines when using CNB in patients with obesity and spinal deformity. Evaluation clinical aspects of symptomatic posttraumatic epilepsy
The article presents the results of a comprehensive drug therapy with melepsin symptomatic post-traumatic epilepsy. We described the comparative changes in the number of epileptic seizures, the analysis of the results of video-EEG monitoring during treatment complex drug therapywith melipsin and traditional methods. The study showed that the therapy with melepsin marked positive changes amplitude frequency, restoration of normal areal distribution of the normal rhythm of bioelectrical activity. Indicators normalized coherence, according to computerized EEG studies after the appointment melepsin revealed a decrease not only intrahemisphericvisual COH in the left hemisphere of the brain, but also reduced the COG of theta activity for long and medium hemispheric oblique temporo-parietal, temporo-occipital, temporo-frontal pairs of leads

Local staging of rectal cancer with conventional and diffusion-weighted magnetic resonance imaging
In this article we present up-to-date views on rectal cancer imaging. Accurate initial and follow-up staging of rectal cancer is vitally important and provides information essential for treatment decision making, preoperative therapy and surgery planning. Almost worldwide magnetic resonance imaging (MRI) is currently considered as the most advanced staging modality for rectal cancer; however conventional MRI sequences have some diagnostic limits and not always allow to differentiate fibrotic tissue (present either desmoplastic reaction or fibrotic changes due to preoperative radiotherapy) from tumor. On the basis of our own experience and published data we tried to demonstrate imaging possibilities of conventional and diffusion-weighted (DW) MRI in local staging of rectal cancer

Blood lymphocytes chromosome karyotyping in patients with endometrial hyperplasia
The article presents data of cytogenetic analysis of chromosomes in blood lymphocytes of women with endometrial hyperplasia and suspected endometrial cancer. By definition of fragile sections of chromosomes were revealed initial stages of expression of mutant genes, often oncogenes, which may indicate the beginning of cancer. It is found that the fragile sites of chromosomes often initiated by chromosomal rearrangements, and later there is chromosomal instability. It is shown that taking into account genetic factors and cytodiagnostics use in women with endometrial hyperplasia has high importance on prognosis, prophylaxis and treatment of endometrial cancer in the early stages of its development.

Comparison analyses of laparoscopic and open pyeloplasty in patients with uretero-pelvic junction obstruction
Comparison of results of laparoscopic (LP) and open(OP) pyeloplasties. We compare retrospectively 35 patients who undergone LP with 50 OP in period from 2010 till 2015y. In comparative analysis of LP and OP we conclude that: mean operative time for LP and OP was 163 min versus 103 (P<0.01), blood loss 31ml versus 132 ml (P<0.01), dosage of used analgesics (diclofenac) after procedure 75mg versus 300 mg (P<0.01), average length of incision 1.6 CM versus 17CM (P<0.01), shortening of hospital stay days 3 versus 8 days (P<0.01), early returning to normal feed state 22 hours versus 30 hours (P<0.01). Intraoperative complications in LP group has been 5.7 % (two patients has been needed for PCN in early postoperative days because of excessive urine leakage from paranephral drain tube) and 6% for OP (3 patients had high temperature which need additional drug requirement).17 patients (34%) in open group had pain in lumbar area “scar pain” after 6th month of operation. In that time only 4(11.4%) of patients from LP had such kind of pain. High effectiveness and low degree of pain in post operative wound makes LP more attractive in treatment of uretero-pelvic junction obstruction

Diagnosis and modern tactics of surgical treatment of iatrogenic injuries of the extrahepatic bile-ducts and external biliary fistulas
The article presents the analysis of surgical treatment of 317 patients with intraoperation damage and external fistulas of the extrahepatic bile-ducts. The reasoning formation of strictures and external biliary fistulas was intraoperative damage during cholecystectomy, gastric resection and echinococcectomy. Preoperatively all the patients underwent correction of violations of water-electrolyte balance, treatment and prevention of hepatic-renal failure, the fight against anaerobic infection. From 317 patients 29 (9.1 %) - restorative and 288 (90.9 %) reconstructive operations were performed. Stricture recurrence that required repeat surgical intervention was marked in 21 (6.7%) of patients. Various postoperative complications observed in 63 (19.8 %). Mortality was 8.7 %. The main cause of death – intoxication, renal failure, associated with suppurative cholangitis and cholangiolitis abscesses of the liver due to insolvency of anastomosis seams
