  • The influence of melatonin on the functional state of the hepatobiliary system with dexamethasone-induced diabetes mellitus was studied. The analysis of numerous research studies made it possible to talk about the high prevalence of complications in diabetes as a disease of the liver, the principal organ of metabolic processes in the body. It was established during the experiment that diabetes reduced the intensity of bile, the number of total bilirubin in it, mainly due to its conjugated forms, cholates and cholesterol, which disturbed the relationship between the components of the bile. The injection of melatonin at a dose of 10 mg / kg intraperitoneally during 10 days lead to an increase in the intensity of bile secretion, increase the concentration of bilirubin in it, both general and conjugate fractions, cholates and cholesterol, improved the processes of conjugation, normalized cholate – cholesterol coefficient.

  • As a result of the study, after two months of treatment in both groups of patients was noted compensation of diabetes and normalization indicators of blood pressure. Improvement of subjective and objective data in the assessment of the neurological status of patients confirms the opinion of the majority of contemporary researchers on the leading role of normalization of glycemia and hemodynamic parameters in the treatment of diabetic polyneuropathy.

  • The aim of the present research was to study the efficacy of Omakor and a - lipoic acid on biochemical indexes of patients with abdominal obesity with manifestations of metabolic syndrome. 105 patients were divided into 3 groups and (hey have been under our observation: the 1st group (30 patients) received traditional therapy, the 2nd group (37 patients) received traditional therapy + Omakor, and the 3rd group (37 patients) received traditional therapy + Omakor and Berlition. The results of the research showed high efficacy in improvement of carbohydrate and Lipid exchange of Omakor and a - lipoic acid combination.

  • The aim of the present work was to study diffuse and nodular goiter prevalence in population of Samarkand Region. 220 patients aged 25-60 years have been studied. The degree of thyroid gland enlargement has been determined by means of palpation and ultrasonic methods. The investigations showed that according to palpation findings diffuse enlargement of TG was revealed in 41.7 of examined patients, diffuse nodular goiter in 3%. USE findings showed diffuse-focal pathology in 15%, nodular masses in 15.5% of all examined patients.

  • Aim of given research was included to study the changes of clinic laboratory indices in prolonged hy-perbilirubinaemias occurring in TORCH in infections. 86 patients at age from 1 to 2 months were examined in the neonatological department of RCHMMC. In all cases hyperbilirubinaemia continued over 14 days. The results of our research showed that the patients were determined evident disturbances of biochemical indices: hyperbilirubinaemia, hypoproteinaemia, hypomagnaemia, hypoglycaemia, increase C- reactive pro-tein, increase of hepatic tests.   • Materials and methods. The study is based on the experience of surgical treatment of 427 patients with liver echinococcosis of various localization , operated in the II summit of the clinic from 2003 to 2015. In 2003-2006 . we operated 154 (36.1 %) patients. These patients were in the control group. The comparison group included 134 (31.4 %) patients operated in 2007-2010. The study group included 139 (32.5 %) patients operated in 2011-2015. Results. Compared with the years 2003-2006 . the frequency of postoperative complications decreased almost 5 times - from 18.4 % to 4.0 %. Relapses echinococcosis late after surgery occurred in 12.9 % of pa-tients and in the group operated in 2003-2006, this figure reached 28.2 %. In subsequent years, against the backdrop of intra- and postoperative prevention of recurrence of the disease in the control group was reduced to 8.8 %, and in the main group reduced to zero.   • The Study have included 80 patients (28.8% -men, 71.2%-women, mean age 58.8± 0.8 years ) with os-teoarthritis (OA) of the knee.   • The study of long-term results of treatment of patients with nodular goiter determined the optimal amount of surgery in different morphological forms of nodular goiter.   • In order to assess the nature and degree of functional disorders of the circulatory system in pregnant women with preeclampsia and obesity in 3rd trimester of pregnancy at 36-38 weeks of gestation were stud-ied 107 women, of whom 73 patients with overweight and obesity during pregnancy complicated by preeclampsia. Comprehensively surveyed patients presented by 3 groups. The 1st group included 48 women with light degree of preeclampsia, the 2nd - 25 patients with severe preeclampsia. 3rd (control) group con-sisted of 34 healthy women of the same gestational age, whose pregnancy considered uncomplicated. Is traced a definite correlation between the degree of obesity and the severity of preeclampsia. It should be not-ed that patients with obesity during pregnancy, which was complicated by of preeclampsia light degree dom-inated hypokinetic circulation mode (70.8%) and in women with severe of preeclampsia, this type of circula-tion occurred in 100% of cases. Hypokinetic type of blood circulation matched hypertensive-hypodynamic dissociation and was characterized by a sharp decline in minute performance of the heart (CI - less than 2.5 liter / (min × m2)), high systemic vascular resistance (over 1500-2000 dynes × sec × cm-5) against the back-ground of arterial hypertension (ADP over 100-110 mm Hg over).   • The article presents the results of a study of 256 pregnant women between the ages of 28 to 40 years old with gestational age from 28 to 41 weeks. All the women had overweight varying severity: overweight (BMI 25-29.9-kg/m 2 ) - at 18, obesity 1st degree (BMI 30-34.9 kg/m 2 ) - at 67, obesity 2nd degree (BMI 35-39.9 kg/m 2 ) - at 96, obesity 3rd degree (BMI - ≥34.9 kg/m 2 ) - at 75 patients. Had conducted a study of the diagnostic and prognostic significance of risk factors for complications. Were conducted replace of group of diagnostic parameters for one integral indicator of high information content. Thus, based on a survey prena-tal pregnant women with overweight and obesity should be based on an integrated system of multivariate prediction of the risk of complications. The integrated system of multivariate forecasting the risk of compli-cations during delivery and anesthesia in patients with obesity allows a high degree of reliability to establish individual risk, timely to conduct the complex therapeutic and preventive measures, to determine the optimal obstetric and anesthetic tactics, eliminate possible tactical mistakes.

  • For hyperthyroidism and hypothyroidism modeling L-thyroxin (200 g/day per 1 kg of body weight) or merkazolil (10 mg/day per 1 kg of body weight) were administered to rats intraperitoneally daily for 14 and 21 days. Administration of L-thyroxin resulted in the increase of serum free thyroxin concentration by 83 and 136%, at the same time the thyroid-stimulating hormone level was significantly decreased (by 56 and 76%). Merkazolil caused the decrease of serum free thyroxine level by 38 and 62% and increase of thyroid-stimulating hormone by 59 and 550%, while the level of free triiodothyronine was decreased by 66 and 74%. Experimental hypothyroidism was accompanied with the increase of cysteine level in blood (by 24% on 14-th day and 39% on 21-st day). Merkazolil resulted also in the increase of homocysteine concentration (by 98 and 160%) and decrease of hydrogen sulphide level (by 17 and 24%). It has been concluded that disorders of sulfur amino acids metabolism could play a pathogenic role in the development of cardiovascular complica-tions in patients with hypothyroidism.

  • The studies conducted in 150 patients operated on for toxic goiter, before surgery and one year after thy-roidectomy. The severity of heart failure (HF) in patients with toxic goiter we studied in terms of six-minute walk test. Before surgery, 48 patients with thyrotoxicosis of average weight at 13, it regarded as HFI and 35 - both HFIIA. One year after the operation structure HF changed: in 5 patients it was not determined, in 21 cas-es was HFI and 22 patients - HFIIA. Among the 102 patients with severe hyperthyroidism, before surgery in 78 cases of HF was defined as HFIIA and 24 - HFIIB. After the operation, 19 cases were diagnosed HFI, 69 -HFIIA and 24 it has not changed. In assessing the long-term results as the tests used change indicators severity of heart failure and cardi-ac arrhythmias, and divided them by the developed gradation, a good, satisfactory, unsatisfactory and ineffec-tive treatment. Good and satisfactory results in patients with thyrotoxicosis of average weight obtained in 47 (98.0%) patients and in patients with severe thyrotoxicosis - in 45 (44.1%). It concluded that long-term results of surgical treatment of patients with toxic goiter are directly dependent on the severity of hyperthyroidism, heart failure and heart rhythm disorders

In the ophthalmic center" A.A.Yusupov ” we examined 18880 patients, 384 (2%) of whom were diagnosed as diabetic retinopathy (DR).Proliferative stage of DR was found in 48.6 % of patients. On average, the rapid development of disease observed in age from 60-65 years. At this age the number of patients with proliferative stage prevailed. The findings suggest the need for and feasibility of population screening programs that may help identify the DR in the early stages of its development.

  • The study was conducted to evaluate options for preoperative preparation for surgical interventions on the thyroid gland. The study included 110patients with thyroid disease age 32 to 49 years. The main group of 55 patients for complex preoperative preparation used adaptol, mexidol, bisoprolol, sibazon; control group of 55 patient sused conventional methods of preoperative preparation. The effectiveness of preopera-tive preparation was assessed by conventional clinical signs, central hemodynamics, who studied at the stag-es of anesthesia and surgery. The study revealed that prolonged sedation use bisoprolol, mexidol adaptol sibazona and ensurereduction of psychological stress, panicandanxiety, stabilization of hemodynamic pa-rameters and the adequacy of the level of sedation.

  • The paper presents results of a study of external breathing parameters in 46 patients with morbid obesity undergoing holetsistektomy preoperatively. Study of external respiration (BH, ML, MML, VC, FVC, IRV, ERV, FEV1, IT, MT). Comparison of produced by sex, age, place of residence and by body weight. It was found that the difference in terms of external respiration by sex and place of residence were not statistically significant, whereas age and body weight showed high reliability. Revealed that with increasing body weight and age of the patient there is a decrease of ML, MML, VC, which should be considered in the preoperative preparation of this category of patients.

  • Results of the laser treatment in patients with diabetic maculopathy complicated by macular edema have showed that the combined using lasercoagulation and peptid bioregulator Retinalamin contributed the resolu-tion of macular edema and allow to enhance of the visual functions. At application of traditional conservative therapy at DM only at 11,4% visual acuity, in 6 months after treatment doubtfully improved, visual acuity was authentically below initial. As a result of the combined treatment of DM by means of laser photocoagu-lation and Retinalamin's preparation visual acuity improved, with 0,140 to 0,333, changes of the area of rela-tive and absolute scotomas at DM, during treatment wasn't noted, the reached effect remains more than 6 months that speaks about stabilization of function of a retina.

  • On the basis of studying of the remote results of treatment of patients with a nodal craw the optimum volume of surgical intervention at various morphological forms of a nodal craw is determined. Adequate volumes of expeditious treatment are the gemistrumektomiya, extremely subtotal resection of a thyroid gland and a tireoidektomiya. When carrying out adequate replacement tireoidny therapy the number of recurrence after the carried-out surgeries sharply decreases. The greatest percent (4,7%) of recurrence irrespective of a morphological form of a nodal craw is revealed when carrying out an economical resection and enukleation of knot of a thyroid gland.

  • The study of trace element status in diffuse toxic goiter appropriate, depending on the functional activity of the thyroid gland. Investigated a number of elements that are "goitrogenic" or "strumogen" factors - the essential trace elements such as selenium, cobalt, copper, manganese and others. The study included 51 pa-tients with diffuse toxic goiter. The survey included: general clinical research study of thyroid status, study trace element status (determination of the content in the hair of patients with iron, selenium, manganese, co-balt, copper, zinc). The studies revealed deficiency of trace elements (iron, manganese, cobalt, copper, zinc) in diffuse toxic goiter, as well as the ability of hair to reflect long-term trends in trace element status of the body allow us to consider the above micronutrient deficiency as a risk factor for diffuse toxic goiter.
